Step 1: Screening
Screening is the first half of the dose form. You record the child’s measurements, who came with them, any medical history, and decide whether the child can receive the drops today. This page is for health workers (GeneralUser) and anyone else who doses children.
The form header shows the child’s name (a link to their profile), UPC and gender. Below it two numbered circles show where you are: Phase 01 Screening and Phase 02 Prashan.
Panels above the form
Section titled “Panels above the form”These appear before any input and are for reading, not typing.
Recommended Dosage (yellow box). The number of Drops for this child’s age, the Batch Code in use, Child Age in months, Dose # (the number this dose will be, counting paper doses too) and, when the child has paper doses, Prior (paper). If no rule matches the child’s age, a red box replaces it: This child is not eligible due to age, with the allowed range and the child’s age. You cannot complete a dose for that child; the server refuses it too.
Dose Rules Reference. The full age table: Min Age (mo), Max Age (mo), Drops, Remarks. It is the same table an admin maintains in Admin → Dose Rules.
Previous Doses. One row per dose the child has already received on this system: Dose, Date, Batch, Location (the camp day number) and Status. Paper doses are not listed here; they are in the Prior (paper) count.
Two other banners can appear at the top:
- Already dosed for this campday (blue). The child has a completed dose on this camp day. The whole form is read-only. See Step 2: Giving the dose.
- 60-dose limit reached (red) or Final dose (amber, at dose 60). See the cap on the same page.
Measurements
Section titled “Measurements”Six boxes in a grid. Type numbers only; decimals are fine (95.5).

If the BMI is low for the child’s age the BMI box turns red and says Likely underweight — consider referral. Under 5 years the flag is BMI below 13. It is a prompt to you, nothing more; the form does not stop you dosing.
None of the measurements is enforced by the form. Fill in what you measured. The site’s own protocol says which ones are compulsory.
Accompanying Guardian
Section titled “Accompanying Guardian”Required. The dropdown lists every guardian linked to the child as Name (Relation). If the child has one guardian, it is pre-selected.
If the person at the desk is not in the list, pick Other (not in list). Two boxes appear, Name and Relation (for example Uncle, Neighbour, Teacher). Both are required. This records who was present today without adding them as a permanent guardian. If they are going to bring the child regularly, add them as a guardian from the child’s profile later.
Medical History & Exclusion Criteria
Section titled “Medical History & Exclusion Criteria”One list of checkboxes, two kinds of item. The note above the list says it: Items marked with the warning sign withhold today’s dose. The rest are recorded as history only.
- Exclusion criteria are shown in red with a warning mark. They come from Admin → Exclusion Criteria and mean “do not give the drops today”, for example a fever or an acute illness.
- History items are plain: Myopic, Malnourished, Under Medication, Allergies, Recent Surgery, Autism, Others. Ticking one records it on today’s session and feeds the medical-history chart in the camp-day report. On its own it does not stop the dose.
- Others (red, with warning mark) is the exclusion for any reason not in the list. Ticking it opens an amber box with Reason for exclusion (text, required) and Adversity Level (Mild, Moderate or Severe, required).
The free-text box under the list is for extra detail on history: conditions, allergies, medications.
When a child is excluded
Section titled “When a child is excluded”As soon as you tick any red item, the form changes:
- Exclusion Remarks appears. Write the reason in a sentence. Required.
- A confirmation checkbox appears: Guardian has been informed about the reason of exclusion, and they have been referred to respective medical professional. Required.
- The Next: Prashan button is replaced by a red Filter Out & End button. The batch code and paper-entry boxes disappear, because no dose will be given.
Press Filter Out & End. The session is saved with status Filtered out, locked, and no dose is recorded. The child’s dose count does not change. The camp-day report counts them under Excluded and the reason appears in the Why Excluded chart. You then see the success screen; press Return to intake.
Batch code and paper entry
Section titled “Batch code and paper entry”Below the checkboxes, when the child is not excluded:
- Batch Code. With one batch on the camp day it is shown fixed. With several it is a dropdown and you must choose. Details in Step 2: Giving the dose.
- Recording a dose given earlier (paper entry). Leave this unticked on a normal Prashan day. It is only for typing in a dose that was given on an earlier day. Read Recording a dose given earlier before using it.
NEXT: PRASHAN
Section titled “NEXT: PRASHAN”The black Next: Prashan → button is disabled until:
- an accompanying guardian is picked (and, for Other, name and relation are typed);
- a batch code is chosen when there is more than one;
- a dose date is chosen when the paper-entry box is ticked, and it is not in the future;
- the child is under the 60-dose cap.
Pressing it does two things. It stamps the screening end time, and it opens a dose session with status Started on the server with everything you typed. The session belongs to you (the logged-in user) and to this camp day. Then the form moves to Phase 02 Prashan.
