Anusuchi-2 (child dose card)
Anusuchi-2 (अनुसूची-२) is the official Ministry of Health and Population register for the Swarna Bindu Prashan service. It has one block per child with the child’s details, the guardian’s details and every dose the child has received. This page is for SuperAdmin, GeneralAdmin and LocationAdmin.
What the export contains
Section titled “What the export contains”Open Admin → Reports → Anusuchi-2 — Recording Register. The file you download has one block for each child with:
- Name, UPC, gender, date of birth.
- Guardian name and phone, district, palika, ward and tole.
- Latest recorded weight and height.
- Every completed dose the child has had at any camp site, oldest first, up to 48 doses, with the adverse effect noted for each.
- Dose numbering starts after the child’s paper doses. A child with 3 paper doses has their first digital dose printed as dose 4.
Column 6, the caste code (जाति कोड), is left blank on purpose. Staff fill it in by hand on the printed register.
Step 1: choose a filter mode
Section titled “Step 1: choose a filter mode”- Mode A — Children list — hand-pick children. Best for a few certificates or for one family.
- Mode B — Campsite & period — everyone dosed at one camp site between two dates. Best for the register of a whole camp or month.
Both modes export the child’s full history across all sites; the mode only decides which children are included.
Mode A: pick children
Section titled “Mode A: pick children”- Type at least two letters of a name, or a UPC, in Search by name or UPC…. The list searches as you type, or click Search.
- The first 50 matches appear, each with name, UPC, gender and the number of doses recorded. If there are more, a line says Showing first 50 of N. Refine the query to narrow down. Type more of the name to shorten the list.
- Click + Add on a child. It changes to ✓ Added and the child appears as a black chip under Selected for export.
- Search again for the next child. Your chips stay while you search.
- Click × on a chip to remove a child.
A LocationAdmin only finds children who have had a session at one of their own sites. A child from another site does not appear in the search, and cannot be exported even if the address is edited.
Mode B: campsite and period
Section titled “Mode B: campsite and period”- Pick a Campsite. A LocationAdmin sees only their own sites.
- Set From (AD) and To (AD). The default is the last 90 days.
- Read the Preview: N children dosed at Site between From and To, with Female and Male totals split into new and repeat.
A child is included when they have at least one completed dose at that site on a camp day whose date falls in the range. Both dates are inclusive.
Step 2: format and download
Section titled “Step 2: format and download”Pick a format from the drop-down next to the button, then click ⇣ Download XLSX
or ⇣ Download PDF. The button shows Generating… while the server works. The
file is named anusuchi-2_<date-time>.xlsx or .pdf.

The button is disabled until there is something to export: at least one child in Mode A, or a preview above zero in Mode B.
The header date
Section titled “The header date”The form has a date cell (मितिः) at the top. The page fills it with today’s date as
2083-01-10 (BS) / 2026-04-23 (AD). The BS date is worked out in your browser. If the
conversion is not available, the cell shows only the AD date.
The 10,000 child cap
Section titled “The 10,000 child cap”One file can hold at most 10,000 children. In Mode A the Selected for export line shows (at the 10000 cap) and refuses more. In Mode B the preview shows a red line, Filter exceeds the 10000-child cap. Narrow the date range before exporting, and the download button stays disabled. Split the period into months and export each.
Common errors
